South Eastern Nigeria faces one of the most acute blood supply crises in Sub-Saharan Africa. A region of approximately 21.8 million people spread across five states - Abia, Anambra, Ebonyi, Enugu, and Imo - loses thousands of lives annually to conditions that are entirely preventable with adequate blood availability: postpartum haemorrhage, sickle cell disease crises, surgical emergencies, road traffic accident trauma, and anaemia from malaria and malnutrition.
The World Health Organization estimates that Nigeria requires approximately 1.8 million units of blood per year to meet minimum healthcare needs, but collects fewer than 500,000 units - less than 28% of what is needed. In South East Nigeria, the gap is even more pronounced: regional blood collection capacity is estimated at approximately 42,000-58,000 units annually against a minimum requirement of approximately 220,000 units, leaving a deficit of more than 75%.
This crisis does not stem from a lack of willing donors. Studies across the South East consistently find latent willingness to donate among a significant proportion of the adult population when the infrastructure, information, and access are available. The crisis stems from the near-total absence of functional blood banking infrastructure: cold chain systems that can maintain blood viability at 2-6 degrees Celsius from collection through storage to transfusion; certified laboratory screening systems for HIV, Hepatitis B, Hepatitis C, and malaria; trained transfusion medicine professionals; and reliable, geographically distributed collection and distribution networks that can reach hospitals when and where blood is needed.
Public health investment alone has not resolved this crisis. Federal and state government blood banking programmes are chronically underfunded, understaffed, and dependent on outdated equipment. The National Blood Service Commission (NBSC), established by the National Blood Transfusion Service (NBTS) Act, has a mandate to coordinate blood transfusion services nationally but operates with resources that are a fraction of what the scale of the problem requires.
It is in this context that Public-Private Partnership (PPP) models emerge as the most viable pathway to rapidly scaling blood banking infrastructure to meet demand - leveraging private capital, operational efficiency, and technological capability within a framework of public oversight and social protection.
